TITLE VI COMPLAINT FORM

ADA Notice Under The Americans with Disabilities Act

In compliance with Title II of the Americans with Disabilities Act of 1990 (ADA), the Northside Youth and Senior Service Center (NSYSSC) ensures that no qualified individual with a disability is discriminated against in its services, programs, or activities. NSYSSC also upholds non-discriminatory practices in hiring and employment, adhering to all regulations set by the U.S. Equal Employment Opportunity Commission under Title I of the ADA. To promote effective communication, NSYSSC will provide necessary aids and services, such as sign language interpreters and Braille documents, to ensure equal participation for individuals with disabilities. Reasonable modifications to policies and procedures will also be made to guarantee access to all programs, services, and activities. Those needing auxiliary aids or policy modifications should contact the program supervisor at 314-531-1937 prior to the event. While NSYSSC is committed to accessibility, it is not required to make changes that would fundamentally alter its programs or impose undue financial or administrative burdens. Complaints regarding accessibility can be directed to the Executive Director, Mr. Leon Threat, at 4120 Maffitt Ave., St. Louis, MO 63113, or via email at lthreat@nsyssc.com. NSYSSC will not impose additional charges on individuals with disabilities for the provision of necessary aids, services, or reasonable policy modifications.

Title VI Language Assistance Plan

The purpose of this Language Assistance Plan is to fulfill the Federal Transit Administration’s (FTA) requirements under Title VI of the Civil Rights Act of 1964, which prohibits discrimination based on race, color, or national origin. As a recipient of FTA funds, this transit system is committed to taking reasonable steps to ensure meaningful access to its services for individuals who do not speak English as their primary language and have limited English proficiency (LEP). This plan aligns with the FTA Circular 4702.1B, which outlines Title VI requirements and guidelines for recipients, and was developed using the U.S. DOT’s FTA Office of Civil Rights’ publication, “Implementing the Department of Transportation’s Policy Guidance Concerning Recipients’ Responsibilities to Limited English Proficient Persons – A Handbook for Public Transportation Providers,” dated April 13, 2007.
